* Applies knowledge of nutritional screening and assessment of specific patient populations and their nutritional needs. Able to appropriately individualize and formulate total parenteral nutrition (TPN) orders for specific patient populations.
* Assists infusion team in monitoring patient status by evaluating TPN formula (macronutrients and micronutrients), lab trends, and nutritional parameters.
* Evaluates nutrition care plan for effectiveness and patient response as well as identification of nutrient deficiencies.
* Contributes to initial and ongoing infusion pathway and care conferences as part of the interdisciplinary team and monitors patient progress toward collaborative home nutrition goals.
* Appropriately communicates patient updates and recommendations to physician and other members of the healthcare team.
* Uses professional judgment to adapt and apply guidelines, protocols, and professional standards of care to new and changing needs of patients.
* Serves as a resource to medical staff and allied health professionals in specialty nutrition area.
* Appropriately documents patient care activity, dietitian services provided, and all communication with patient, physician, and health care team.
* Documentation is accurate, complete, and complies with company policy and procedure for electronic and paper patient clinical record entries. Conducts ongoing chart review per policy.
* Forwards required documentation to physician, payers, and referral sources within policy or contractually stated timelines.
* Provides sales team with clinical updates to generate and support referral relationships.
* Educates staff as appropriate on Medicare pre qualification for home nutrition support.
* Determines whether patient meets Medicare guidelines for coverage of home TPN therapy or whether patient should be billed to Medicare for denial or if an Advanced Beneficiary Notice is required.
* Obtains all supporting documentation and evaluates TPN prescription in accordance with Medicare guidelines.
* Works with revenue management from start of care throughout entire billing process to ensure prescriptions contain all necessary information for prompt payment.
* Performs in-service education to internal customers, referral sources, physicians and nursing agencies.
* Provides instruction on clinical topics related to disease-state management, improvement of patient outcome, and reimbursement issues.
* Serves as a mentor to students and staff in specialty nutrition area.
* Identifies learning needs of patient and/or caregiver.
* Communicates patients learning needs and findings to health care team members, including other clinicians, customer service staff, and physician/referral source, as appropriate.
* Provide diet planning/modification and nutrition education of oral diet to patient/patient family when appropriate.
* Participates in education and training conferences on home nutrition care.
* Participates in training sessions of sales personnel marketing the nutrition program.
* May assist sales personnel in clinical aspects of marketing home nutrition programs.
* Performs other related duties as directed by supervisor.
* Registered Dietitian with active registration by the Commission of Dietetic Registration of the American Dietetic Association.
* Bachelor's degree from an accredited college or university in Foods and Nutrition/Dietetics or related field.
* Completion of approved coordinated undergraduate program, dietetic internship, or AP-4 program required.
* Advanced certification recognized by the National Board of Nutrition Support Certification of the American Society for Parenteral and Enteral Nutrition - Certified Nutrition Support Dietitian (CNSD). If not currently CNSD at time of application 4 yrs related experience may be considered and CNSD certification is then required within 2 years of employment.
* Current License in appropriate state of practice is required.
