Job Description

 
1.1. TASKS/DELIVERABLES:

The contractor shall provide one FTE to serve as a Nurse Case Manager to support to GPRMC, Fort Sam Houston, Texas. The contracted employee must be a highly qualified individual who also possesses a. high level of personal and corporate access to analytical skills, tools, and resources in support of GPRMC Regional requirements. Program support shall include performing the following functions:
• Interact with GPRMC at all levels to understand their unique requirements and develop
strategies to support and prioritize those requirements
• Forecast NCM requirements.
• Utilize various DOD, Army and Medical Department computer programs and other databases
as needed
• Receive, interpret, disseminate, and implement various policy, regulation, directives, and
guidance from the Army Medical Command
• Design or acquire methods for data collection and presentation
• Interact with peers and appropriate agencies outside the GPRMC to accomplish WTU mission.

• Participate or lead in conferences, meetings, VTCs, telecon, etc as required
• Perform quality assurance duties
• Validate data
• Prepare briefings, reports, bullet papers, or other supporting documentation as needed
• Advise Executive Staff on. status and implementation of programs
• Advises commanders and staff in all aspects of nurse case management.
• Assists medical staff in evaluating quality of patient care documentation and in developing
criteria and methods for such an evaluation.
• Advises the medical commanders on issues pertaining to patient movement.
• Provides necessary consultation on matters pertaining to Joint Commission accreditation
standards.
2.3. EDUCATION AND TRAINING REQUIREMENTS. On-site contractor personnel shall be able to demonstrate their expertise through a combination of education and experience. A bachelor's degree in nursing and a current Texas licensure are required. Three years or more of demonstrated knowledge in DOD a health care management setting.
